- Voters who were on the roll in 2002 must fill the first box with 2002 information; if not, parents’ 2002 voter details go in the second box.
- If 2002 details are unavailable, the voter may leave both boxes blank, sign the form, and respond later with any of the 11 identity documents.
- For fresh inclusion, Form 6 requires a declaration and any one of the 12 listed documents, according to the CEO.
- For address change, Form 8 can be filed in August 2026; the voter may then appear in the final roll, not the draft roll.
